Key takeaways

  • Mayo pays physicians a step-based salary with no productivity incentives or bonuses, a model it has used for more than 40 years. Non-salary pay and benefits are the same across its locations and specialties.
  • Minnesota now requires pay ranges in job postings, and Mayo's show the same $217,500 to $533,750 full-time starting base for a hospitalist and a hospital psychiatrist. That is Mayo's salary band, not a specialty quote; your step within it is the number to negotiate.
  • Mayo still offers a pension at no cost to employees. Physicians who became eligible on or after January 1, 2023 earn it under a Stable Lump Sum formula, paid as a lump sum at normal retirement.
  • The 403(b)/401(k) match is 50 to 100 cents per dollar on the first 4% of pay, rising with years of service: $7,000 to $14,000 a year on a $350,000 salary. A 457(b) is also reported.
  • Mayo is a 501(c)(3) nonprofit, so direct full-time employment generally counts toward PSLF.

Compare

Most physician offers start with a base salary and then spend pages on the part that moves: the wRVU threshold, the conversion factor, the quality bonus. A Mayo Clinic offer doesn't have that part. The salary is the compensation, and it rises in steps.

That makes Mayo offers unusually easy to compare and unusually hard to research, because Mayo, like Cleveland Clinic, doesn't publish what it pays by specialty. Two things have changed that. Minnesota's pay transparency law now puts a salary range on every Mayo job posting in the state, and Mayo's own retirement plan documents spell out how its pension works for physicians hired today.

This guide pulls those sources together: what the posted ranges actually mean, what the self-reported and visa-filing data show, how the pension and 403(b) match work, and which questions to ask before you sign. It's part of our physician employer guides, alongside Kaiser, the VA, HCA and the university systems.

The salary-only model

Mayo describes its physician pay as a structured, step-based salary, in place for more than four decades. There are no incentives tied to volume and no opportunity for bonus pay. Mayo adopted it to remove the financial incentive to do more than a patient needs, or less. Non-salary compensation and benefits are consistent across Mayo's locations and specialties.

Mayo tested the model on itself. In a study published in Mayo Clinic Proceedings at the end of 2019, its researchers reviewed all 2,845 permanent staff physicians in clinical roles in Arizona, Florida and Minnesota as of January 2017. Pay matched the model's prediction for 96% of them. The 80 physicians paid more than predicted and the 35 paid less showed no correlation with gender, race or ethnicity, and most of those outliers had blended or unusual departmental appointments.

For a physician weighing an offer, the model has three practical consequences:

  • No upside from volume. A proceduralist who would out-earn their base elsewhere through productivity doesn't capture that here. Our wRVU guide explains what you'd be giving up.
  • No downside from volume, either. A slow ramp-up, a new practice building referrals or a reduced schedule after a leave doesn't cut pay the way a productivity formula can.
  • The negotiation is about placement. With no bonus structure to argue over, the questions that matter are which step you start on, how fast steps advance and what the top of the scale is for your specialty.

What Mayo physicians earn

Since January 1, 2025, Minnesota law has required employers with 30 or more employees to post a good-faith pay range, and a description of benefits, in every job posting. Mayo's Minnesota physician postings now carry one, and they reveal something about how Mayo pays.

SourceFigureWhat it tells you
Mayo posting: Hospitalist, Austin, MN$217,500–$533,750"Minimum starting salary," full-time total base before experience or added duties
Mayo postings: hospitalist nocturnist (Mankato) and adult hospital psychiatrist (Rochester)$217,500–$533,750The same band for different specialties
Mayo postings: academic hospitalist (Rochester) and anesthesiologist (Austin)$211,250–$533,750A slightly lower floor, same ceiling
Glassdoor, Mayo "Physician"$242,132–$427,467 typical range (150 reports)Self-reported, all specialties and locations mixed
Glassdoor, Rochester physiciansAbout $247K–$384K; median near $308K (50 reports)Self-reported, Rochester only
H-1B wage filings, "Senior Associate Consultant"Median about $399,861 (20 records, mostly 2022–2025)Actual offered wages, but a small sample of visa-sponsored hires

Read the posted range as Mayo's salary scale, not your offer. When a hospitalist posting and a hospital psychiatrist posting list the identical $217,500 to $533,750, the range isn't describing either specialty. It spans Mayo's steps and specialties, and the posting itself says pay for the selected candidate varies with specialty, experience, FTE, internal equity and external market data. A hospitalist is unlikely to start near the top, and a high-paying surgical specialty is unlikely to start near the bottom.

The self-reported and visa-filing numbers fit that picture. Glassdoor's middle range of roughly $242,000 to $427,000 sits well inside the posted band, and the H-1B median near $400,000 reflects a sample tilted toward subspecialists. For context on where your field sits nationally, compare against our physician salary by specialty data, keeping in mind that national averages include productivity pay that Mayo doesn't offer.

The useful move: pull up the current posting for your specialty and site on Mayo's careers page, note the range, and ask recruiting which step you'd start on and what the step progression looks like for your specialty over the first five years.

Senior associate consultant to consultant

Mayo's staff physician titles are its own. New staff physicians are typically appointed as senior associate consultants, the title on many current postings, and later become consultants. Mayo doesn't publish a standard timeline. One gastroenterologist's published CV shows three years as a senior associate consultant before becoming a consultant, but that's a single example from decades ago, not a rule.

Because the title change can come with a change in salary step, it belongs on your list of questions: how long the senior associate period lasts in your department, what the criteria are, and what your salary does at the transition.

The pension

Mayo describes itself as one of the few U.S. employers still providing a pension benefit at no cost to employees. For a physician, that's retirement money that doesn't come out of your paycheck and doesn't count against your 403(b) limits.

How it's calculated depends on when you became eligible. Mayo's plan summary describes three formulas:

FormulaWho it applies toHow it pays
Stable Lump SumParticipants who became eligible on or after January 1, 2023, and earlier participants who chose it in the Mayo Retirement Choice program starting January 1, 2024A lump sum at normal retirement date
Annual AccumulationService from January 1, 2015 for earlier participants who didn't switch (the default)Builds a benefit year by year
Final Average PayService through December 31, 2014 for participants eligible before 2015Based on pay near the end of the career and years of service

Three rules apply across all of them. Benefit service is capped at 30 years. Most participants vest after three years of full-time service, according to Mayo, though the plan document sets the exact rule for your situation. And a physician hired today lands in the Stable Lump Sum formula, so the benefit is best thought of as a growing pot of employer money paid out at retirement rather than a monthly check for life.

Mayo doesn't publish the accrual rates in its public summary, so ask for the Stable Lump Sum formula itself and an illustration at your expected salary. That number, and the three-year vesting point, are what to weigh against a competing offer with a richer 401(k) match but no pension. If you leave before vesting, the pension is worth nothing to you.

The 403(b) match and 457(b)

On top of the pension, Mayo matches contributions to its 403(b) and 401(k) plans. Its careers site describes the match as 50% to 100%, depending on accrued participation, and its job postings give the formula: for every dollar you contribute, Mayo matches the first 4% of pay at $0.50, $0.75 or $1.00, depending on years of service.

Match tierOn a $350,000 salaryOn a $500,000 salary
$0.50 per dollar on 4%$7,000$7,200
$0.75 per dollar on 4%$10,500$10,800
$1.00 per dollar on 4%$14,000$14,400

Illustration. Matches above $360,000 of pay are capped by the 2026 IRS compensation limit, which is why the $500,000 column barely rises. Mayo's plan documents set the service years for each tier.

To get the full match you need to contribute at least 4% of pay yourself. Most physicians will want to contribute the full $24,500 employee deferral for 2026 anyway; our retirement contribution limits guide has every current limit, including catch-ups.

The 457(b). A financial planning firm that works with Mayo employees lists a 457(b) deferred compensation plan among Mayo's retirement benefits. If you're eligible, it adds a second $24,500 of tax-deferred room in 2026. At a private nonprofit, though, 457(b) money stays the employer's asset until it's paid out, and payout rules can be strict, so ask for the plan's eligibility rules and distribution options before you raise your election. Our open enrollment guide covers that tradeoff.

Rochester, Arizona or Florida

Mayo's three major campuses are in Rochester, Minnesota; Phoenix and Scottsdale, Arizona; and Jacksonville, Florida. The Mayo Clinic Health System adds community hospitals and clinics in Minnesota, Wisconsin and Iowa. Mayo reported nearly 85,000 employees after adding more than 12,400 in 2025.

Because Mayo's non-salary benefits are the same across locations, the biggest location difference for your finances is state income tax. Florida has none, Arizona has a low flat rate, and Minnesota's top bracket is among the highest in the country. On a physician salary that gap can run into five figures a year. Our take-home pay guide shows how much state tax changes what you keep at the same salary.

One more detail: Minnesota's posting law covers Minnesota jobs, so Arizona and Florida postings may not show a range. Ask for it.

PSLF

Mayo Clinic is a 501(c)(3) nonprofit, and full-time work for a 501(c)(3) employer generally counts toward Public Service Loan Forgiveness. That can make a Mayo salary worth more than a higher offer from a for-profit group if you carry large federal loans. Look up the employer identification number (EIN) on your W-2 in the PSLF Help Tool, and certify every year; our guide to checking whether your hospital qualifies walks through it.

The tradeoffs

  • Mayo vs. Cleveland Clinic. Both pay a straight salary with no productivity incentive. The retirement plans differ: Mayo has a no-cost pension plus a 50–100% match on 4% of pay, while Cleveland Clinic contributes 2.5% to 5.5% of salary to an account-based plan and matches 50% of the first 6%. Our Cleveland Clinic guide has the details.
  • Mayo vs. a productivity employer. Compare Mayo's salary with the other offer's realistic total pay at typical volume, not its base. Then add Mayo's pension and match, which a for-profit group often can't match. Our HCA guide shows the productivity side.
  • The pension rewards staying. The three-year vesting point and the 30-year service cap both favor a long career at Mayo. If you expect to leave within a few years, value the offer mostly on salary and match.
  • Little room to negotiate pay outside the scale. A step-based system leaves less room for one-off deals. Signing bonuses, relocation, start dates and step placement are where to focus; our contract negotiation guide covers the rest.

Questions to ask Mayo recruiting

  1. Which salary step will I start on, and what does the step progression look like for my specialty over five and ten years?
  2. How long is the senior associate consultant period in my department, and what changes in pay when I become a consultant?
  3. Can I see an illustration of my Stable Lump Sum pension benefit at my expected salary, and the vesting rule that applies to me?
  4. How many years of service does each match tier ($0.50, $0.75, $1.00) require?
  5. Am I eligible for the 457(b), and what are its distribution options?
  6. Is malpractice coverage provided, and is there any tail exposure if I leave? (Our tail coverage guide explains why this matters.)
  7. Which Mayo entity will be my W-2 employer, for PSLF purposes?

Frequently Asked Questions

How much do Mayo Clinic physicians make?

Mayo doesn't publish salaries by specialty. Its Minnesota job postings, which must list a pay range under state law, show a full-time starting base of $217,500 to $533,750 for roles as different as hospitalist and adult hospital psychiatrist (a few postings start at $211,250). That range is the institution's band, not a specialty quote. Self-reported Glassdoor data puts the middle 50% of Mayo physicians between $242,132 and $427,467, and H-1B wage filings for senior associate consultants show a median of about $399,861 across 20 records. Ask recruiting where your specialty and experience place you on the scale.

Does Mayo Clinic pay physicians bonuses or wRVU incentives?

No. Mayo has paid physicians a structured, step-based salary with no productivity incentives or bonus pay for more than four decades. Mayo says it adopted the model to remove financial incentives to do more, or less, than a patient needs. Its own study of 2,845 staff physicians in Arizona, Florida and Minnesota found pay equity for 96% of them, with no link between outlier pay and gender, race or ethnicity.

Does Mayo Clinic still have a pension?

Yes. Mayo describes itself as one of the few U.S. employers that still provides a pension benefit at no cost to employees. The formula depends on when you became eligible. According to Mayo's plan summary, participants who became eligible on or after January 1, 2023 earn benefits under a Stable Lump Sum formula paid as a lump sum at normal retirement; earlier participants have Final Average Pay service before 2015 and Annual Accumulation service after. Benefit service is capped at 30 years, and most participants vest after three years of full-time service.

What is Mayo Clinic's 403(b) match?

Mayo's careers site describes an employer match of 50% to 100%, depending on accrued participation. Its job postings give the detail: Mayo matches the first 4% of pay you contribute at $0.50, $0.75 or $1.00 per dollar, depending on years of service. On a $350,000 salary that's $7,000 to $14,000 a year of employer money, on top of the pension. Mayo's own plan documents set the service-year cutoffs for each tier.

Is Mayo Clinic a qualifying employer for PSLF?

Mayo Clinic is a 501(c)(3) nonprofit, and full-time employment directly by a 501(c)(3) generally qualifies for Public Service Loan Forgiveness. Confirm the exact employing entity on your W-2 with the PSLF Help Tool at StudentAid.gov, and certify your employment every year.

What's the difference between a senior associate consultant and a consultant at Mayo?

They are Mayo's staff physician titles. New staff physicians are typically appointed as senior associate consultants and later become consultants. Mayo doesn't publish a fixed timeline; one gastroenterologist's CV shows three years at the senior associate level before consultant. Ask during recruiting how long the senior associate period lasts in your department, and how your salary step changes when you move to consultant.
